Context
medoco HEALTH is a healthcare IT start-up in Trento working for the orthodontic sector. It builds software and devices to capture, archive and share clinical images in dental and orthodontic practices, on open standards: DICOM, FHIR, HL7, PACS.
The platform is web-based and built on microservices: it connects directly to the practice's instruments to capture images and data with no manual entry, catalogues them automatically with AI models, and makes them shareable in DICOM between professionals and devices.
Problem
A complex product to take from prototype to production: image capture from cameras and scanners, conversion to DICOM with clinical metadata, sending to the PACS, patient records via FHIR, a dedicated hardware device, interfaces for staff. A small team, heavyweight standards, zero margin for error on patient data.
Solution
Since January 2025 I've worked on it almost full time as a contractor: architecture, backend and frontend development, DevOps and CI/CD, hardware integration with a dedicated Raspberry Pi device, the DICOM and FHIR standards with SNOMED coding, technical documentation, working with the first users and project management.
With Toni Magni I wrote the FHIR Implementation Guide published by medoco HEALTH: it standardises how AI services that classify images (modality, anatomical region, orientation) integrate, so the images can be converted to DICOM, with no patient data required.
Outcome
The suite has been in production for three years at the pilot practice and is now being installed in new clinics. Numbers: hours saved per week, images archived, patient-matching errors
